From isolationto belonging.
A relationship-first NDIS pathway that helps you build genuine friendships, find your community, and feel connected to the world again — at your own pace, with a worker who understands what it actually takes.
Imagine a life where… you belong somewhere, and you have people.
Not activities for the sake of activities. Not community access as a line item. Real connection — the kind that makes your life feel worth showing up for. Built slowly and patiently, with a worker who gets it.
- You have people to call, people to make plans with, people who actually know you.
- You have a regular activity you genuinely look forward to every week.
- You feel more comfortable walking into a room of new people than you do now.
- You feel like you belong somewhere — a group, a place where you're a familiar face.
- You leave the house not because you have to, but because something out there matters to you.
- The loneliness that used to fill your evenings and weekends starts to lift.
- You're building friendships that exist beyond the support session — because they're real.
From “it feels safer at home” to “I belong here”
Most community support takes you to an outing and reports that you attended. This pathway is designed to move you — from isolation toward a community where you're genuinely known.
- ●Most of your time is spent at home, where it feels safer than going out.
- ●You want friends, but genuinely don't know how to make them.
- ●New social situations make you anxious, so you cancel at the last minute.
- ●You've tried groups before and felt like an outsider the whole time.
- ●Your family is your only social outlet — you don't have anyone your own age.
- ●You go to things sometimes, but come home feeling worse, not better.
- ✓You leave the house for something that genuinely matters to you.
- ✓You're building real friendships — with the same trusted worker beside you.
- ✓Venues are pre-checked and familiar, so anxiety drops visit by visit.
- ✓You've found a group matched to what you love, not whatever had a vacancy.
- ✓You have people your own age who know your name and remember you.
- ✓You come home lighter — and your connection is measured, review by review.
Built for people who are ready to reconnect
This pathway is for you if…
- ✓You spend most of your time at home and want that to change.
- ✓You want friends but genuinely don't know how to make them.
- ✓You get anxious in social situations and tend to avoid them.
- ✓You're autistic, or have a psychosocial disability, and experience social withdrawal.
- ✓You've lost confidence rebuilding social life after hospital, trauma or a big change.
- ✓You're a CALD participant wanting culturally connected community.
It may not be the right fit if…
- —You're actively refusing all community engagement right now.
- —You need clinical mental-health intervention before community support can begin.
- —You're not yet ready to take even one small step toward connection.
If that's where you're at, that's completely okay — call us anyway. Psychosocial recovery support first may be the better starting point, and we'll help you find it.
The things that are hardest to say out loud
We hear the same words from participants and families when they first call. If any of these sound familiar, you're in the right place.
Social isolation is one of the most painful things a person can carry — and one of the hardest to talk about.
The longer it goes on, the more it feeds itself: confidence shrinks, the world feels smaller, and reaching out feels harder than staying in. We don't treat loneliness as a symptom to manage. We treat it as a goal worth working toward — seriously, patiently, and at a pace that never pushes you past where you're ready to be.
You don't have to do this alone anymore.
Four stages. One small step at a time.
Each stage builds on the one before it. We never rush you into a situation you're not ready for — because being pushed before you're ready is exactly what makes social anxiety worse, not better.

Lasting connection
Understand you first
- Interest mapping — what do you love, what have you always wanted to try?
- Social confidence assessment — where are you starting from, honestly?
- Which environments feel okay, and which feel impossible?
- What's been tried before — what happened, what didn't work?
- Cultural and faith preferences — what already feels like home?
Find safe, meaningful opportunities
- Identify 2–3 activities matched to your interests — not generic programs
- Your worker pre-visits new venues to check the environment first
- The first visit is always with your familiar worker — no surprises
- Try at your own pace — one thing, one visit, one small step
- Revisit the same places until they start to feel familiar
Develop confidence and social skills
- Conversation practice in real settings — not role play
- Social coaching in the moment, reading situations and responding
- Navigating the hard moments — silences, new faces, endings
- Familiar people at regular places become known faces
- Practical, personal strategies for managing social anxiety
Build lasting connection
- Strengthen emerging friendships — follow up, make plans, stay in touch
- Establish social routines that exist without support sessions
- Attend with less worker presence as your confidence grows
- Find one or more communities where you feel genuinely known
- Your worker fades as your social world expands
What your first 90 days can look like
This is an example of how the pathway typically unfolds — a composite, not a specific participant. Everyone's plan and pace are different, but the shape of the journey is the same: small, real wins early, then steadily fewer hands holding yours.
We start by understanding you
Your first conversation lands within five business days — relaxed, no questionnaire. We map what you love and what you've always wanted to try, and gently score where your social confidence is starting from. By week's end you have a Connection Goal Plan in your own words, plus 2–3 activities that genuinely interest you.
The first visit — and it isn't as bad as you feared
Your worker pre-visits a venue, then goes with you the first time. You attend a community activity, with no pressure to talk to anyone. You go back a second time, and a familiar face recognises you. By the third visit you know what to expect — and the anxiety is already lower.
Conversations start to happen
With social coaching in the moment, you start a conversation and remember someone's name. You look forward to at least one regular outing each week. The same places, the same faces — it's beginning to feel less like visiting and more like belonging.
You can see what changed
At the 90-day review, your starting scores are re-measured. Social Confidence moved from a 2 to a 4. Community Participation from a 1 to a 3. There's a friendship that now exists beyond the support session — written down, goal-referenced, and ready for your plan review.
Illustrative only. The specific activities, timeframes and progress shown here are an example of how the pathway can work — not a guaranteed outcome. Your journey is built around your interests, your plan and your pace.
We don't just hope you're connecting. We measure it.
Every participant is scored across five connection domains at the start, then re-scored at 30, 60 and 90 days. The gap between your first score and your next is the evidence of your progress — and it's what protects your funding at plan review.
Social Confidence
Comfort initiating and holding conversations, and managing new environments.
Community Participation
How regularly you attend chosen activities — is a weekly rhythm forming?
Social Connections
Known faces, names exchanged, plans made outside the support sessions.
Belonging
Your own felt sense of being recognised and welcomed in a regular setting.
Independence
How much you attend with reduced — or no — worker presence.
Early momentum, on purpose
Connection doesn't happen overnight — but momentum does. So we deliberately design for small, real wins from the very first week.
A Connection Goal Plan and 2–3 activities that genuinely interest you.
Attend a community activity for the first time — and survive it.
Return to the same place; a familiar face recognises you.
Been somewhere three times — you know what to expect, anxiety drops.
These are examples of the kind of early progress participants frequently experience when the match between interests, environment and worker is right — not guaranteed outcomes.
Everything that makes it actually work
Most providers send a worker and an invoice. The pathway is a complete system — assessment, planning, consistency, measurement and reporting — built around you.
- ✓An Interest Discovery & Social Confidence assessmentA relaxed conversation, not a questionnaire — because every activity flows from what you actually love.
- ✓A written Connection Goal PlanYour goals, in your words, shared with your coordinator within two weeks.
- ✓The same 1–2 workers, matched on interests & cultureFor the whole pathway, with a briefed backup — the relationship is the foundation, not an afterthought.
- ✓A Community Opportunity Library matched to youReal local activities for your interests and suburb — not whatever generic program had a spot.
- ✓Your worker pre-visits venues before you goSo the first time is never a cold plunge into the unknown.
- ✓Culturally responsive support for CALD communitiesCultural and faith community connection explicitly supported — Hindi, Punjabi and more, with preferences documented at intake.
- ✓30 / 60 / 90-day measured reviewsClear evidence of your connection growing, every quarter.
- ✓Fortnightly written updatesSo your family and coordinator stay informed without ever chasing.
- ✓A plan-review-ready progress reportGoal-referenced connection evidence your coordinator can use to protect your funding.
The risk is on us
Trying again after past disappointments takes courage. So these aren't vague reassurances — they're commitments in writing, several with a concrete consequence if we fall short.
The Consistency Guarantee
The same 1–2 workers for your whole first 90 days — matched on your interests and cultural background, not just availability.
The Fast-Start Guarantee
Your first session within five business days of you being ready to begin.
The Communication Guarantee
A written progress update every fortnight, and a reply to any message within one business day.
The Interest-Match Guarantee
Every activity flows from your Interest Discovery Profile — never from whatever generic program happens to have a vacancy. A poor interest match is the single biggest reason community participation fails, and we won't make it.
The 30-Day Fit Promise
If within the first 30 days this pathway isn't the right fit, we co-manage your transition to support that is. You are never left without help.
We make ourselves unnecessary
Our job is to help your social world grow until you don't need us beside you — never to become your only social contact. Belonging is the goal, not dependence on a worker.
“I built Anchor & Aim to be the provider I'd want for someone I love — small enough to know your name, serious enough to do the work properly. The guarantees on this page aren't marketing lines. They're the standard I hold every shift to — and if we ever fall short, I want to hear from you directly.”

Not sure what's in your plan?
Call us on 0411 727 133 and we'll go through your NDIS plan with you. We'll explain what's available, what's fundable, and what's realistic given your budget — no jargon, no pressure.
Where you attend small group activities (2–4 participants), costs are split per participant per the NDIS price guide, documented in shift notes, and your plan manager is notified before any group billing begins.
Good to know
How long does the pathway take?
Typically 6–18 months, depending on your starting confidence and the funding in your plan. You move entirely at your own pace — we never push you into a situation you're not ready for, because that's exactly what makes social anxiety worse.
What NDIS funding do I need?
The pathway usually draws on Core Supports (Social, Economic & Community Participation, and Assistance with Daily Life) and Capacity Building (Increased Social & Community Participation, and Improved Daily Living), depending on your plan. We confirm the line items with your plan manager before billing begins.
What if I get too anxious, or want to cancel?
That's completely expected, and it's okay. Your worker pre-visits venues before you go, your first visit is always with a familiar face, and there's no pressure to engage early. A 30-day review and our 30-Day Fit Promise mean you're never stuck.
Do you offer culturally responsive support?
Yes. We explicitly support cultural and faith-based community connection, and we match workers to your culture, faith and language wherever possible — with Hindi and Punjabi-speaking workers among those available.
Can I do activities in a group?
The pathway always starts one-to-one. As your confidence grows, there's an optional transition to small groups of two to four participants — only when and if you're ready.
Where do you provide the pathway?
Across Melbourne's eastern and south-eastern suburbs, delivered one-to-one out in your community.
What people are saying
I wanted to join a local gaming group but walking into a room full of strangers alone makes me feel completely frozen. My worker didn't force me to just go in and start talking to people. We literally just sat in the back for the first two sessions until I got used to the noise. Now I actually look forward to Tuesdays.”
DanielParticipant
A lot of providers think community access just means driving a participant to a shopping centre to kill two hours. What I appreciate here is that they actually focus on the relationship aspect. They put thought into matching the right worker, and they treat building social confidence as a real skill that takes time.”

Before this, she spent almost all her time in her bedroom scrolling on her phone. It was breaking my heart because she was so lonely but just didn't know how to break the cycle. Seeing her get dressed up and genuinely excited to go meet a friend for a coffee last week... honestly, I had to hide my tears.”

You don't have to explain why you're lonely
We already understand. Just tell us a little about what you'd like — a regular outing, a group to try, a place to belong. We'll do the rest. Finding the right match between you, an activity, and a worker who gets it is what we're good at.
Enquiring for a family member who's isolated or withdrawn? You don't need to have it all figured out — just tell us what you're seeing. We know how hard it is to watch someone you love shrink away from the world. We get back to you within 2 business hours.
